概述

銀屑病是一種常見的以紅斑鱗屑為特征的慢性炎癥性皮膚病。

病因

與遺傳、感染、變態反應、代謝障礙、自身免疫等有關。許多研究認定本病具有遺傳性，傢族血緣關系明顯高於普通人群，遺傳方式和編碼發病信息的基因位點不清楚。有人提出本病受多基因控制，與自然界生態環境（如氣候）、人工環境三者相結合導致發病。發病常與病灶感染、內分泌障礙、代謝障礙有關，有的患者可在外傷或預防接種後發生。精神緊張、季節變換、寒冷潮濕、飲酒常為誘發因素。

癥狀

1、本病的損害，初期為點滴狀棕紅色斑點或斑丘疹，表皮覆蓋著多層銀白色鱗屑，以後逐漸擴展，相鄰損害可互相融合或擴大，境界清楚。

2、損害可發生在身體任何部位，尤以四肢伸側、肘膝關節、頭皮和骶骨部位為常見。頭皮部損害為斑丘疹，有銀白色鱗屑覆蓋，邊緣清楚，頭皮受損害牽制頭發而聚成束狀，但不脫落，指趾甲也可發生變化，甲板上有針頭大小的凹坑或甲板發生溝紋或甲板增厚，污褐，表面平，呈鉤甲。手掌及足底，偶然引起角化過度及皸裂。陰莖龜頭也可發生境界明顯的紅色斑塊，鱗屑往往很少。粘膜上發疹極少。 檢查

1、血常規、尿常規、肝功能與轉氨酶、胸透、心電等。

3、組織病理檢查。 治療

治療原則為：一般療法，避免飲酒及其他刺激性食物，勿濫用刺激性、過敏性藥物；去除可疑病因，如慢性感染灶等；應用皮質類固醇激素治療；應用免疫抑制劑療法治療；應用中藥辯證施治；應用物理療法；應用外用藥物療法。

1、尋常性銀屑病：

（1）發病與感染病灶有關者，靜脈滴註白黴素，成人60萬單位加5%葡萄糖註射液500ml，每日1次，20日為1療程。

（2）發病與神經精神有關者或劇癢者，可用靜脈封閉療法，成人用0.25%普魯卡因100～200ml加5%葡萄糖250ml，緩慢靜脈滴註，每日1次，20日為1療程。使用本療法時應先作皮試，對磺胺過敏者慎用此法。

（3）同時口服乙亞胺，每次0.1g、1日4次，定期查肝功能與白細胞數，白細胞數低於正常值時停藥。加服鯊肝醇等藥。

（4）維生素療法① 維生素b12500μg。1日1次、肌肉註射，1個月為1療程。② 維生素ad，1次2丸，1日3次口服。

（5）局部外用：皮質類固醇激素霜，如恩膚霜等，皮損停止發展，炎癥減輕後，可外用1/2萬芥子氣軟膏或0.5%蒽林軟膏，每日1～2次。

2、紅皮病、膿皰與關節病型銀屑病：

（1）皮質類固醇激素療法：一般僅用於紅皮病型、關節病型或泛發性膿皰型銀屑病，且使其他藥無效者。尋常性銀屑病禁用。

氫化考的松註射液200mg或地塞米松註射液10mg加5%葡萄糖註射液500ml、維生素c5.0g，10%氯化鉀10ml，靜脈滴註，每日1次，待皮損基本治愈後，可開始逐漸減量。

（2）免疫抑制劑：若皮質類固醇激素療效不佳或出現減量困難時，可加用免疫制劑，如環磷酰胺註射劑200mg加0.85%氯化鈉註射液20ml，靜脈註射，1周2次，連續10次為1療程。或口服硫唑嘌呤50mg，1日3次，4周為1療程。

（3）非激素類抗炎藥物：關節病型銀屑病，可口服消炎痛1次50mg、1日3次。昆明山海棠片，1次2片、1日3次。

（4）物理療法：①沐浴療法。礦泉浴如能同時采用紫外線或日光照射（夏季型銀屑病患者禁用），效果更好。在浴液中加入適當藥物（如硫酐或松餾油等）可增加加治療作用；②光化學療法（puva）；對久治不愈的尋常型銀屑病和對一般治療無效或因副作用嚴重而不能繼續治療的紅皮病型及膿皰型銀屑病患者，可考慮采用本療法。
